REMINDER: Supplements can carry traces of banned substances that are not on the label. It is the student-athletes responsibility to check with Athletic Training Staff if the supplement is okay to consume. Should a student-athlete test positive for an NCAA drug test due to supplements regardless of reporting it, they hold sole responsibility.

This NCAA Drug Consent Form must be signed by ALL student-athletes prior to the beginning of their season, including pre-season. This form will be collected by the Athletic Training staff, but these forms will be held with the Athletic Director and Compliance.*
